Principles and Best Practices for Oral History–The Oral History Association’s guide on interviewing people in the course of your research

Oral History Consent Form : Use this to make sure you gain documented consent from each of the people you interview. Once you have signed, completed forms, send a digital copy to Leah so they can be stored at the COPLAC office.

Digital Image Publication Permission Form: If you gain permission from the creator of a photograph, video, etc. to publish their work on your site, use this form to document that permission. Once you have signed, completed forms, send a digital copy to Leah so they can be stored at the COPLAC office.
